The following is a checklist to help you gather information for law enforcement if your identity was used to file a fraudulent income tax return or if you believe that you may be a victim of identity theft.

The Department is committed to assisting taxpayers who are the victims of identity theft, and continually strives to increase data security to stop fraudulent returns before they are processed. Due to our rigorous screening process, taxpayers who are victims of identity theft may experience longer than normal processing times for their tax returns. We understand that identity theft is a frustrating process for victims, and we are diligently working to combat the issue and protect taxpayers.

Was your electronic return rejected by the IRS?
If your electronic return was rejected by the IRS, please take the following steps:
- Report the identity theft to us using our web form below or if you do not have access to a computer by calling (515) 281-5986. We will put an ID theft alert on your account for added protection.
- File your Iowa return either electronically, if your tax software permits, or in paper form by mailing it to the Department of Revenue at this address:
Iowa Income Tax Document Processing, PO Box 9187, Des Moines, IA 50306-9187
NOTE: Include a copy of your federal return, all W2s and 1099s, and any necessary schedules. If you are filing by paper you may also attach a copy of your IRS Identity Theft Affidavit (form 14039) to your Iowa return.
